Protecting your private information is our priority. This Statement of Privacy applies to https://www.youscript.com, https://www.youscript.net, and YouScript Inc. and governs data collection and usage. For the purposes of this Privacy Policy, unless otherwise noted, all references to YouScript Inc. include https://www.youscript.com, https://www.youscript.net, and YouScript. By using YouScript websites, you consent to the data practices described in this statement.
Collection of your Personal Information
YouScript may collect personally identifiable information, such as your name. YouScript may also collect anonymous demographic information, which is not unique to you, such as your age and gender. We may gather additional personal or non-personal information in the future.
Information about your computer hardware and software may be automatically collected by YouScript. This information can include your IP address, browser type, domain names, access times and referring website addresses. This information is used for the operation of the service, to maintain quality of the service, and to provide general statistics regarding use of the YouScript website.
This site has implemented Google’s Invisible reCAPTCHA. Use of Invisible reCAPTCHA is subject to Google’s Privacy Policy and Terms of Service.”

Use of your Personal Information
YouScript collects and uses your personal information to operate its websites and deliver the services you have requested.
YouScript may also use your personally identifiable information to inform you of other products or services available from YouScript and its affiliates. YouScript may also contact you via surveys to conduct research about your opinion of current services or of potential new services that may be offered.
YouScript does not sell, rent or lease its customer lists to third parties.
YouScript may share data with trusted partners to help perform statistical analysis, send you email or postal mail, provide customer support, or arrange for deliveries. All such third parties are prohibited from using your personal information except to provide these services to YouScript, and they are required to maintain the confidentiality of your information.
YouScript may keep track of the websites and pages our users visit within YouScript websites in order to determine what YouScript services are the most popular. This data is used to deliver customized content and advertising within YouScript to customers whose behavior indicates that they are interested in a particular subject area.
YouScript will disclose your personal information, without notice, only if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to the edicts of the law or comply with legal process served on YouScript or the site; (b) protect and defend the rights or property of YouScript; and, (c) act under exigent circumstances to protect the personal safety of users of YouScript, or the public.
Collection of Protected Health Information
We may use and/or disclose Protected Health Information (PHI) in the same manner as Personal Information, described above, except our use and disclosure of Protected Health Information is further limited as provided by the administrative simplification provision of the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), the Health Information Technology for Economic and Clinical Health Act of 2009 (“HITECH”) and the Omnibus regulations promulgating Standards for Privacy of Individually Identifiable Health Information and Security Standards for the Protection of Electronic Protected Health Information promulgated thereto.
Specifically, as described above, all uses or disclosures of PHI shall require patient authorization or a valid authorization on the patient’s behalf, except: (1) uses or disclosures by or to the Patient; (2) uses or disclosures for treatment, payment or healthcare operations; (3) as part of any valid use or disclosure; or (4) in compliance with and pursuant to Applicable Law.
We will enter into business associate agreements with the Patient’s Providers who are “Covered Entities” when we are a “Business Associate,” as those terms are defined by HIPAA. We will use and disclose Protected Health Information only for those uses and disclosures permitted by HIPAA and under the applicable business associate agreement. We may use or disclose Protected Health Information to provide Services to the Patient or the Provider. We may also use Protected Health Information for our proper management and administration or to carry out our legal responsibilities.
You have the following rights regarding the PHI maintained by YouScript:
Confidential Communication. You have the right to receive confidential communications of your PHI. You may request that YouScript communicate with you through alternate means or at an alternate location, and YouScript will accommodate your reasonable requests. You must submit your request in writing to YouScript.
Restrictions. You have the right to request restrictions on certain uses and disclosures of PHI for treatment, payment or healthcare operations. You also have the right to request that YouScript restrict its disclosures of PHI to only certain individuals involved in your care or the payment of your care. You must submit your request in writing to YouScript. YouScript is not required to comply with your request. However, if YouScript agrees to comply with your request, it will be bound by such agreement, except when otherwise required by law or in the event of an emergency.
Inspection and Copies. You have the right to inspect and copy your PHI. You must submit your request in writing to YouScript. YouScript may impose a fee for the costs of copying, mailing, labor and supplies associated with your request. YouScript may deny your request to inspect and/or copy your PHI in certain limited circumstances. If that occurs, YouScript will inform you of the reason for the denial, and you may request a review of the denial.
Amendment. You have a right to request that YouScript amend your PHI if you believe it is incorrect or incomplete, and you may request an amendment for as long as the information is maintained by YouScript. You must submit your request in writing to YouScript and provide a reason to support the requested amendment. YouScript may, under certain circumstances, deny your request by sending you a written notice of denial. If YouScript denies your request, you will be permitted to submit a statement of disagreement for inclusion in your records.
Accounting of Disclosures. You have a right to receive an accounting of all disclosures YouScript has made of your PHI. However, that right does not include disclosures made for treatment, payment or healthcare operations, disclosures made to you about your treatment, disclosures made pursuant to an authorization, and certain other disclosures. You must submit your request in writing to YouScript and you must specify the time period involved (which must be for a period of time less than six years from the date of the disclosure). Your first accounting will be free of charge. However, YouScript may charge you for the costs involved in fulfilling any additional request made within a period of 12 months. YouScript will inform you of such costs in advance, so that you may withdraw or modify your request to save costs.
Breach Notification. You have the right to be notified in the event that YouScript (or a YouScript Business Associate) discovers a breach of unsecured PHI.
Paper Copy. You have the right to obtain a paper copy of this Notice from YouScript at any time upon request. To obtain a paper copy of this notice, please contact YouScript by calling (877) 796-4362.
Complaint. You may complain to YouScript and to the Secretary of the Department of Health and Human Services if you believe that your privacy rights have been violated. To file a complaint with YouScript, you must submit a statement in writing to YouScript: Attn: Compliance Officer, 108 S Jackson St #300, Seattle, WA 98104. YouScript will not retaliate against you for filing a complaint.
Use of Cookies
The YouScript websites may use “cookies” to help you personalize your online experience. A cookie is a text file that is placed on your hard disk by a web page server. Cookies cannot be used to run programs or deliver viruses to your computer. Cookies are uniquely assigned to you, and can only be read by a web server in the domain that issued the cookie to you.
One of the primary purposes of cookies is to provide a convenience feature to save you time. The purpose of a cookie is to tell the Web server that you have returned to a specific page. For example, if you personalize YouScript pages, or register with YouScript, a cookie helps YouScript to recall your specific information on subsequent visits. When you return to the same YouScript website, the information you previously provided can be retrieved, so you can easily use the YouScript features that you customized.

If you choose to decline cookies, you may not be able to fully experience the interactive features of the YouScript services or websites you visit.
Use of Google AdWords
This website uses the Google AdWords remarketing service to advertise on third party websites (including Google) to previous visitors to our site. It could mean that we advertise to previous visitors who haven’t completed a task on our site, for example using the contact form to make an inquiry. This could be in the form of an advertisement on the Google search results page, or a site in the Google Display Network. Third-party vendors, including Google, use cookies to serve ads based on someone’s past visits to the YouScript website. Of course, any data collected will be used in accordance with our own privacy policy and Google’s privacy policy.
You can set preferences for how Google advertises to you using the Google Ad Preferences page, and if you want to you can opt out of interest-based advertising entirely by cookie settings or permanently using a browser plugin.
Security of your Personal Information
YouScript secures your personal information from unauthorized access, use or disclosure. When personal information is transmitted to other websites, it is protected through the use of encryption.
Children Under Thirteen
YouScript does not knowingly collect personally identifiable information from children under the age of thirteen. If you are under the age of thirteen, you must ask your parent or guardian for permission to use this website.
Opt-Out & Unsubscribe
We respect your privacy and give you an opportunity to opt-out of receiving announcements of certain information. Users may opt-out of receiving any or all communications from YouScript by using “unsubscribe” links in our mailings and/or contacting us.
